(1) According to Section 4 of the Act, health institutions wishing to provide kidney transplant services shall submit an application to the committee in the format as per Schedule-2 along with the form as per Schedule-1 for a permit to do so.

(2) After receiving the application according to sub-rule (1), the committee shall carry out the necessary investigation regarding whether the physical facilities, manpower and other necessary facilities as per Schedule-3 have been arranged in the health institution giving such an application and whether such manpower has acquired the qualifications as per Schedule-4 and 5. If it is found to be the case during such investigation, the committee will grant a license to such health institution to operate kidney transplant services in the format as per schedule-6.
