: (1) When filing a complaint in accordance with sub-section (1) of section 36 of the Act in relation to any action taken by a product or service provider in violation of the Act or this regulation, the following shall be disclosed in the format as per Schedule-9:- < br> (a) the name and address of the person or organization who has acted in contravention of the Act or these Regulations,
(b) The details of the defect in the goods produced or sold or the service provided by the person or organization as per clause (a) or the loss or damage caused by his/her actions,
(c) Name, address and contact number of the complainant,
(d) Evidence of information to confirm the conduct of the complaint.
(2) When filing a complaint according to sub-rule (1), it can also be filed through electronic means (fax, email, telephone, etc.).
(3) Notwithstanding anything contained in sub-rule (1), if the complainant wishes to keep his name confidential, he shall disclose the reason thereof in the complaint.
(4) Action on any complaint shall not be barred merely because it is not in the format as per sub-rule (1).
