Name of doctor or health worker to be listed
:
(1) Every doctor or health worker who wants to provide services must get his name listed in the list.
(2) Physicians or health workers who wish to have their name listed in the list must submit an application to the concerned authority in the format as per schedule-1 along with the listing fee specified by the department.
(3) If an application is received in accordance with sub-section (2), the concerned officer shall, within seven working days, list the name of such physician or health worker in the list established in the format as per Schedule-2 and give a certificate listed in the format as per Schedule-3.
(4) Every doctor and health worker who has received the listed certificate according to sub-section (3) of the Civil Act relating to life, 28 B. No. shall be deemed to have obtained a license to provide services for the purpose of .
(5) Listed certificates obtained by doctors or health workers should be posted in the place where they work so that everyone can see them.
