: (1) When the investigating officer submits an application to the Juvenile Court for the permission to be placed in surveillance room in accordance with sub-section (7) of section 21 of the Act, the child who has been detained on the charge of criminal acts shall be given in the format as per Schedule-3. (2) Application received according to sub-rule (1): After examining the above, the Juvenile Court allows such children to be placed in the monitoring room in the format as per Schedule-4 or if it is not found appropriate to keep them in the monitoring room, according to sub-section (5) of section 21 of the Act, their father, mother, family Other members, custodians, organizations or children's correctional homes can be ordered in the format as per Schedule-5.
