(1) If the identity card taken in accordance with this Act is lost, destroyed, inoperable, if the information on the visible part of the identity card needs to be changed, or if the identity card cannot be used for other reasons, the person concerned shall submit an application to the local authority in the prescribed format to get a copy of the identity card.
(2) If an application is received in accordance with sub-section (1) and the case is found to be genuine during the investigation, the local authority shall send it to the department along with a recommendation and the department shall take the prescribed form and provide him with a copy of the identity card.
(3) Other arrangements regarding the copy of identity card shall be as prescribed.
