(1) After completing the inspection, inquiry or search according to rules 3 and 4, the inspector shall submit a written report to the manager within three days from the date of completion of the work.
(2) In the report submitted in accordance with sub-rule (1), the inspector shall specifically mention the following matters:-
(a) the place and reason for such inspection, investigation and search;
(b) What kind of medicine has been found in such a place and what kind of materials, tools and equipment related to that medicine have been used and what kind of pharmacists, businessmen and people are involved in the production, sale, distribution, transportation and storage of such medicine,
(c) the reason for the seizure or seizure of the drug found;
(d) If the sample of medicine has to be taken, the reason why it has to be taken,
(e) When the medicine is stopped or the sample of medicine is taken, how much medicine has been kept and sealed in rent and for what purpose such rent has been given to whom?
