(1) For the purpose of Section 7 of the Act, a physician who wishes to provide kidney transplant services shall attach the application form as per Schedule-1 and the certified copy of the certificate of competency as per Schedule-4 for the personal license as per Schedule-7. The application shall be submitted to the committee in the following format.

(2) After the application is submitted in accordance with sub-rule (1), if the committee conducts the necessary examination and finds that such physician has obtained the qualification as per Schedule-4, the applicant physician shall be referred to the applicant physician for the purposes of Section 8 of the Act. Will provide individual license to provide kidney transplant services in the following format.
