According to the Act, if a qualified doctor wants to register his name, he must submit an application as per Schedule 1 to the Council.
(2) When submitting an application according to sub-rule (1), it should also be written which type of registration is desired out of the following-
(a) Permanent registration,
(b) Provisional registration,
(c) test registration, or
(d) Expert registration.
(3) When making an application according to sub-rule (1), the following certificates should also be attached and filed along with the application form-
(a) The copy of the certificate of the degree on the basis of which it is desired to be registered and a copy certified by the doctor appointed by the council from among the members of the council or registered doctors shall be returned after the registration of such certificate or the decision not to register.
(b) 3 copies of passport size photograph of applicant taken within 6 months prior to application,
(c) Copies of certificates of citizenship and other certificates,
(d) Affidavit signed by the applicant to remain loyal to the profession as per schedule-2
(e) M.B.B.S. or B.D.S. Or a copy of the degree passed by him at the time of taking admission in a similar course and one copy attested by a doctor appointed by the council from among the members of the council or registered doctors.
(f) Other documents prescribed by the Council.
