The persons whose names are registered in the register book according to sub-section (2) of section 14 or who apply for registration under sub-section (1) of the same section shall provide the following evidence to the registrar , details and information must be given:-

(a) Degrees, certificates of educational qualification and other proofs and documents deemed necessary by the council,

(b) any medical council or council or so If his/her name is registered in the registration book of the organization, details of his/her qualification or title, registration number and date of opening, (c) Affidavit that he/she will perform the duty according to this Act and Rules.< /p>
