Rule 55
Details to be recorded
(1) The child welfare officer shall record and keep the following documents and details related to children in need of alternative care in his personal file:-
(a) In case of death of both parents in the case of orphan children as per clause (a) or (t) of sub-section (1) of section 4D of the Act, death registration certificate issued by the local district magistrate,
(b) According to section (b) of sub-section (1) of section 48 of the Act, in the case of children left unattended in the hospital, the letter from the hospital and the public notice issued to search for the parents of the children and in the case of the children left or found abandoned in a public place according to the same section, the relevant police office 8 Public notice issued to search for recommendation letters and parents of children,
(c) In the case of children under clause (c) of sub-section (1) of section 48 of the Act, the recommendation of the local level and certificate of severe disability,
(d) Order, judgment or letter of the relevant court in the case of children as per clause (d) of sub-section (1) of section 48 of the Act,
(g) In the case of children as per clause (e) of sub-section (1) of section 48 of the Act, the letter of the concerned prison and the consent of the parents, T) In the case of children as per clause (f) of sub-section (1) of section 48 of the Act, the related complaint or the said behavior A letter from the child welfare officer certifying it and the petition filed before the child welfare officer, etc.). In the case of children as per clause (g) of sub-section (1) of section 48 of the Act, the report of the social worker appointed as per sub-section (2) of section 62 of the Act and the petition filed before the child welfare officer,
(h) Children's family: details, property details, . Documents and certificates related to education and health,
(i) Details of relatives, families, individuals, family-based care institutions or children's homes as per sub-section (2) of section 49 of the Alternative Care Act,
(N) The decision of the child welfare officer and the opinion of the social worker according to rule 54.
(2) When arranging for alternative care, the child welfare officer shall arrange for the child's birth registration if he does not have a birth certificate.
(3) 8 Personal files of children sent for alternative care shall be recorded by the respective local level.
