(1) For the purpose of updating the name registration record, every registered health practitioner shall submit to the Council the details of the format prescribed by the Council.
(2) The health professionals shall submit the statement form as per sub-rule (1) to the council for a fee of fifty percent of the fee mentioned in rule 4.
(3) The names of health professionals who do not submit or file the details form and fee as per sub-rule (1) and (2) will not be included in the updated list and they cannot be appointed or nominated to any position in the Council.
