: (1) The insured shall choose the service provider from which service provider he/she will receive treatment services at the time of joining the program. (2) In accordance with sub-rule (1), the insured shall receive health care from the service provider as the first service point of his choice.
(3) If the insured is unable to obtain the required service from the service provider as per sub-rule (1), he shall only receive the service from the service provider sent on the basis of the dispatch parts to be provided to him from such service provider.
(4) Notwithstanding anything contained in sub-rule (2), in case of emergency, the insured may take service from any service provider.
(5) If the insured needs to change the service provider chosen according to sub-rule (1), the registered officer himself or through the registered assistant: before Schedule-3. The application should be submitted in the following format.
(6) Within three days of receiving the application according to sub-rule (5): The registration officer shall change the service provider and inform the concerned applicant.
