Rule 6
Retained health insurance policy
: (1) The board shall issue an insurance policy in the format as per schedule-2 through the registered assistant in the name of each member of the family affiliated to the program and provide a separate identity card to each insured.
(2) If the identity card is destroyed, torn or lost due to any reason, the board shall reissue the identity card to the concerned insured after taking a fee of fifty rupees.
(3) When the registration assistant provides the identity card according to sub-rule (1). All the information about the service facility that the insured will get during the period should be given.
(4) The identity card provided as per sub-rule (1) shall be valid for one year from the date of service. However, if the amount within the prescribed limit for the facility available under the program does not reach one year, he will not be allowed to take any other service facility available under the program until the remaining period of one year.
