Sir/Madam,
We, the following couple have already concluded the marriage/tied the knot of marital relationship on ………(date) in accordance
with our religious, ethnic or dynastic custom, usage, tradition or practice and at present we are living as a husband and wife, we ,hereby, are
interested to register our marriage/marital relation pursuant to the Act; therefore, we are submitting this Application for the same.
Registration of our marriage shall not be in against of the Nepal laws and Section 4 of the Act. We request to register our marriage/maritalrelation pursuant to the Act. We, hereby, agree to bear the legal consequences, in case, the particulars submitted, hereunder, are found false.
