जैविक पदार्थको उत्पादन, विक्री वितरण वा पैठारी गर्न निवेदन दिँदा उपलव्ध
गराउनु पर्ने विवरण
(१) उत्पादन गर्नेको नाम र ठेगाना(Name and address of the producer)

(२) जैविक पदार्थको व्यापारिक तथा जेनेरिक नाम (Trade and generic name of thebiologicals)
(३) जैविक पदार्थको भौतिक तथा रसायनिक बनावट तथा जैविक गुण र प्रभाव गर्ने
तरीका (Physical, chemical and biological properties and mechanism of action of the biologicals)
(४) जैविक पदार्थको प्रतिकारात्मक, उपचारात्मक वा रोग पहिचानको गुणात्मक विवरण
(Description of the prophylactic, curative or diagnostic properties of the biologicals)
(५) जैविक पदार्थमा हानीकारक असर नभएको रोग प्रतिकारात्मक, असर तथा अन्य
